You must have your name start with a capital letter. That is, it should be Name and not name. Furthermore, I will definitely not accept something like this: nAmE/NaMe.
I don't require real or full names. As long as rule #1 is followed, we'll get along well.
Please provide a valid e-mail address.
If you don't own a website, please leave its field empty. Don't put "Coming Soon!" or any of that sort on it. An update form is always available. Additionally, no dead links as inputs.
On the other hand (4), if you do have a website, you may put a link to this fanlisting on it. If you want, you can just grab some of the buttons in the codes page.
